Who qualifies

  • Utah resident
  • Diagnosed with diabetes and using insulin
  • Built for uninsured or high-deductible patients (no income test)
  • Not a state employee or dependent with insulin coverage through PEHP

  • Utah residents (any insurance status)
  • Diagnosed with diabetes and prescribed insulin
  • Discount card valid at participating Utah pharmacies

Not accepted

  • Out-of-state residents
  • Non-insulin medications
  • State employees or dependents with insulin coverage through PEHP

How to apply

  1. Visit utahinsulin.net and complete the online application
  2. On approval (typically a few days), receive your program ID card
  3. Bring the ID card + your insulin prescription to a participating Utah pharmacy
  4. Pay the discounted rate (originally $30/month, adjusted annually for inflation)
  5. Renew annually
