Who qualifies

  • Live in Minnesota
  • Have an urgent need for insulin — less than a 7-day supply with risk of significant health consequences
  • Have a current prescription for insulin
  • Pay more than $75 each month for your insulin
  • Present a valid MN ID, driver’s license/permit, tribal-issued ID, or ITIN (parent/guardian must provide if under 18)
  • Not currently enrolled in Medical Assistance or MinnesotaCare

Accepted

  • Minnesota residents only
  • Less than a 7-day supply of insulin remaining (urgent need)
  • Pay more than $75/month for insulin
  • Have a current insulin prescription

Not accepted

  • Currently enrolled in Medical Assistance (MA) or MinnesotaCare
  • Already used the Urgent Need program in the last 12 months (one supply per year)

How to apply

  1. Confirm you live in MN, have a current insulin Rx, and pay more than $75/month for insulin
  2. Download the application PDF and fill out page 2 (or get a copy at the pharmacy)
  3. Bring it to any MN pharmacy with valid MN ID, driver’s license, tribal ID, or ITIN, plus your Rx
  4. Pharmacist verifies on the spot — leave with a 30-day supply for ≤$35
