Who qualifies

  • Live in Minnesota (valid MN ID required)
  • Household income ≤400% of federal poverty guidelines
  • Not enrolled in Medical Assistance or MinnesotaCare
  • Not on a plan that already caps insulin cost-sharing at ≤$75 / 30 days
  • Medicare Part D enrollees: must also spend ≥$1,000 of household income on Rx that year

Accepted

  • Minnesota residents (any insurance status, including uninsured)
  • Household income ≤400% federal poverty guidelines
  • Prescribed Lantus, Toujeo, Apidra, or Admelog

Not accepted

  • Enrolled in Medical Assistance (MA) or MinnesotaCare
  • On a plan with insulin cost-sharing already ≤$75 / 30 days
  • Non-Sanofi insulins (apply to the matching manufacturer instead)

How to apply

  1. Download the Minnesota application from sanofipatientconnection.com (or call 1-888-847-4877)
  2. Submit application + proof of MN residency to Sanofi by fax
  3. Sanofi notifies you within 5–10 business days
  4. Once approved, Sanofi ships the insulin directly to your home (~5–7 business days)
  5. Renew annually
