Who qualifies

  • Live in Minnesota (valid MN ID required)
  • Household income ≤400% of federal poverty guidelines
  • Not enrolled in Medical Assistance or MinnesotaCare
  • Not on VA prescription drug benefits
  • If insured, you pay more than $75 out-of-pocket for a 30-day insulin supply
  • Medicare Part D enrollees qualify only after $1,000+ Rx spend that calendar year

Accepted

  • Minnesota residents (any insurance status, including uninsured)
  • Household income ≤400% federal poverty guidelines
  • Prescribed Novolog, Fiasp, Levemir, Tresiba, or Novolin

Not accepted

  • Enrolled in Medical Assistance (MA) or MinnesotaCare
  • On VA prescription drug benefits
  • Non-Novo insulins (apply to the matching manufacturer instead)

How to apply

  1. Download/fill the Novo Patient Assistance application — patient, prescriber, or MNsure navigator can submit
  2. Submit via the NovoCare portal or call 1-866-310-7549
  3. Novo notifies you of the decision by phone + letter
  4. Once approved, Novo ships a 120-day supply of insulin directly to your home
  5. Renew annually
