Who qualifies

  • Live in Minnesota with valid proof of residency (MN ID, driver’s license, tribal ID, or ITIN)
  • Household income ≤400% of federal poverty guidelines
  • Not enrolled in Medical Assistance or MinnesotaCare
  • Not on VA prescription drug benefits
  • If insured, you pay more than $75 for a 30-day insulin supply
  • Medicare Part D enrollees qualify only after $1,000+ Rx spend that calendar year

  • Minnesota residents only
  • Household income ≤400% federal poverty guidelines
  • Prescribed Humalog, Lyumjev, Basaglar, or Humulin

Not accepted

  • Enrolled in Medical Assistance (MA) or MinnesotaCare
  • On VA prescription drug benefits
  • Non-Lilly insulins (apply to the matching manufacturer instead)

How to apply

  1. Call the Lilly Minnesota PAP at 855-447-8412 or apply via the Lilly state programs portal
  2. Mail/fax in proof of MN residency, income, and prescriber info
  3. On approval Lilly mails you an eligibility letter — bring it to your pharmacy with the Rx
  4. Lilly ships the 90-day supply to your pharmacy; pay ≤$50 copay at pickup
  5. Renew annually

After Lilly approves the patient, present the eligibility letter + Rx to the pharmacy. Lilly ships a 90-day supply to the pharmacy; collect ≤$50 copay at dispensing.
