  • Live in Minnesota (valid MN ID required)
  • Household income ≤400% of federal poverty guidelines
  • Not enrolled in Medical Assistance or MinnesotaCare
  • Not on VA prescription drug benefits
  • If insured, you pay more than $75 for a 30-day insulin supply

Not accepted

  • Enrolled in Medical Assistance (MA) or MinnesotaCare
  • On VA prescription drug benefits
  • Non-Biocon insulins (apply to the matching manufacturer instead)

How to apply

  2. Submit application + proof of MN residency, income, and prescriber info
  3. Biocon notifies you of approval and ships insulin to your pharmacy or prescriber
  4. Renew annually
