Who qualifies

  • US resident
  • Household income ≤ 400% FPL (≤200% FPL for Ozempic uninsured)
  • Uninsured OR Medicare Part D (excluding Ozempic 2026 onward)
  • Not enrolled in Medicaid, LIS/Extra Help, or VA Benefits
  • Has a Novo Nordisk Rx and an HCP signature on the application

Accepted

  • US resident, age 18+
  • Household income at or below 400% FPL (≤200% FPL for Ozempic uninsured)
  • Uninsured, or Medicare Part D (Medicare Part D NOT eligible for Ozempic as of 2026)
  • Has a current Novo Nordisk Rx

Not accepted

  • Commercial insurance with adequate Novo Nordisk drug coverage
  • Households above the applicable FPL limit
  • Enrolled in or eligible for Medicaid, Medicare LIS / Extra Help, or VA Benefits
  • Medicare Part D beneficiaries seeking Ozempic (2026 exclusion)
  • Wegovy, Rybelsus, and Victoza (not on the 2026 PAP product list)
  • Non–Novo Nordisk medications

How to apply

  1. Visit diabetespap.novocare.com and submit the patient portion online
  2. Provide your healthcare provider's email — they'll receive a secure link to complete their portion
  3. If prompted, upload supporting docs: Proof of Income, Medicaid LIS Denial, Government ID
  4. If approved, Neovance Specialty Pharmacy (1-800-488-5908) will call to schedule home delivery

Using it at the pharmacy

This is patient assistance — Novo Nordisk's Neovance Specialty Pharmacy (1-800-488-5908) ships the medication directly to the patient's home. No pharmacy handoff once approved.

Income tiers by product/insurance: (1) Most Novo Nordisk products: ≤400% FPL. (2) Ozempic UNINSURED: ≤200% FPL only. (3) Ozempic Medicare Part D: NOT ELIGIBLE — Medicare patients should use the Medicare Prescription Payment Plan (MPPP) instead. (4) Medicare patients on Novo Nordisk INSULINS: ≤400% FPL, but if income is below 150% FPL you must provide proof of denial for Part D Extra Help. Uninsured patients in states with high FPL thresholds must provide proof of Medicaid denial before enrollment. Wegovy, Rybelsus, and Victoza are NOT covered by the PAP. Levemir was discontinued by Novo Nordisk in the US on Dec 31, 2024 and is no longer in the PAP. Document ID NNIPAP_11_01012026.
