  • Has health insurance (commercial, Medicare, or Medicaid)
  • Diagnosed with Type 2 diabetes

Not accepted

  • Uninsured patients
  • Above 300% FPL income
  • Currently closed to new applicants — check status before applying

Once approved, HealthWell pays your insurance copay/coinsurance directly. Provide them your pharmacy info during application.
