All terms — $35 copay, 14 fills/yr (cash and commercial-without-coverage), 16 fills/yr (commercial-with-coverage), $3,000/month max, $16,000/year max, 12/31/2026 expiration, MA/CA carveouts, AFP exclusion — are inherited from LIVP. Use lilly_insulin_value for matching and detail.
