  • Humalog (insulin lispro) Group 2 — household income ≤ $63,840 (1-person, 400% FPL)
  • Lyumjev (insulin lispro-aabc) Group 2 — ≤ $63,840 single, 400% FPL
  • Basaglar (insulin glargine) Group 2 — ≤ $63,840 single, 400% FPL
  • Humulin (human insulin) Group 2 — ≤ $63,840 single, 400% FPL
  • NOT in Lilly Cares formulary (2026): Mounjaro, Zepbound, Rezvoglar

2026 income limits (single-person household): Group 1 ≤ $47,880 (300% FPL — Trulicity, Cialis, Emgality, Forteo, Reyvow); Group 2 ≤ $63,840 (400% FPL — all Lilly insulins: Humalog, Lyumjev, Basaglar, Humulin); Groups 3–4 ≤ $79,800 (500% FPL — non-diabetes products). Higher household sizes scale up. Alaska/Hawaii — call for limits. Enrollment duration: uninsured = 12 months; Medicare Part D = expires at calendar year-end. Trulicity is once again accepting new applications (May 2026 — no longer requires medical exception). Mounjaro, Zepbound, and Rezvoglar are NOT in the Lilly Cares formulary as of 2026.
