This is patient assistance — once approved, Dexcom ships sensors directly to the patient. No pharmacy handoff. Patient pays Dexcom directly after approval.
Major eligibility constraint: ONLY for Type 1 Diabetes patients (T2D not eligible) who take daily insulin. Patient must have already checked Dexcom coverage at their local pharmacy AND through a medical supplier before applying. Income limit: ≤ 400% National Poverty Level. Age 2+ (children eligible). Short-term support only — up to 6 months of supplies per calendar year (typically 2 shipments of 90-day supplies). Subject to availability. NOT for Medicare/Medicaid/VA/TRICARE patients — they need to use the Medicare DME or state benefit path instead. Reopened to new enrollees 1/1/2026.
