Call CareConnect4Me at 1-866-279-8990 before assuming a copay card is active. Historically: submit to RxC Acquisition d/b/a RxCrossroads by McKesson using BIN #610524 as a secondary claim after primary insurance.

IMPORTANT — verification gap. The only public Trijardy XR T&C found (BI document PC-US-135219, 09/23) showed $10/month copay with $175 max savings per 30-day supply, and explicitly expired 12/31/2024. As of 2026-05-11, the Trijardy XR consumer site has no Savings tab and BI has not published a successor T&C at the same URL. Jardiance + Synjardy were renewed to a new $0/$100/$450 template (10/2025) — Trijardy may have followed but it cannot be independently confirmed. BIN 610524 (RxC Acquisition / RxCrossroads by McKesson) was the historical vendor.
