  • Xigduo XR (dapagliflozin + metformin XR) — CLOSED to new enrollees 5/1/2026
  • AIRSUPRA, BEVESPI, BREZTRI (non-diabetes — for asthma/COPD)
  • LOKELMA (sodium zirconium cyclosilicate, non-diabetes — for hyperkalemia)
  • Onglyza (saxagliptin) — DRUG DISCONTINUED (not just PAP)
  • Kombiglyze XR (saxagliptin + metformin) — DRUG DISCONTINUED

Who qualifies

  • Uninsured OR Medicare-only (without LIS / Extra Help)
  • Annual income ≤300% FPL (≤500% FPL for Wainua)
  • Not on any other patient-assistance program

  • Annual adjusted gross income ≤300% FPL (Primary/Specialty drugs) or ≤500% FPL (Rare Disease — Wainua only)
  • Uninsured OR Medicare beneficiary not eligible for/enrolled in Extra Help (LIS)
  • Not receiving any other medication assistance

Not accepted

  • Have any commercial (private or employer) insurance
  • Have government insurance other than Medicare (e.g., Medicaid, VA, TRICARE)
  • Medicare beneficiaries eligible for / enrolled in Medicare Extra Help (LIS)
  • Receiving any other medication assistance program
  • Income above 300% FPL (Primary/Specialty)
  • New applicants for Farxiga or Xigduo XR after 5/1/2026 (existing enrollees may continue)

  2. Take the Eligibility Assessment, then download the application for your specific medication
  3. Patient + prescriber complete and sign — prescriber can also ePrescribe directly to the AZ&Me pharmacy

This is patient assistance — AstraZeneca ships your medication directly to you or your prescriber. No pharmacy handoff. Prescribers can ePrescribe directly to the AZ&Me pharmacy.

2026 changes: (1) Farxiga and Xigduo XR closed to NEW enrollees 5/1/2026 — current enrollees can continue refills, but new applicants are turned away. Uninsured Farxiga/Xigduo XR patients should instead use the Farxiga Family Savings Card cash-pay path ($150/month off — see az_farxiga). (2) Brilinta removed from AZ&Me entirely 1/1/2026. (3) Onglyza + Kombiglyze permanently discontinued by AZ (drugs themselves, not just PAP). Eligibility: US resident, treated by US-licensed HCP, must NOT have any commercial or government insurance other than Medicare. Medicare beneficiaries: must NOT be eligible for or enrolled in Medicare Extra Help / Low-Income Subsidy (LIS). Cannot be receiving other medication assistance. Income limit ≤300% FPL for primary medications including Farxiga/Xigduo XR. Other diabetes-relevant AZ&Me drugs (non-diabetes): AIRSUPRA, BEVESPI, BREZTRI (COPD/asthma), LOKELMA (hyperkalemia).
