This is patient assistance — AstraZeneca ships your medication directly to you or your prescriber. No pharmacy handoff. Prescribers can ePrescribe directly to the AZ&Me pharmacy.
2026 changes: (1) Farxiga and Xigduo XR closed to NEW enrollees 5/1/2026 — current enrollees can continue refills, but new applicants are turned away. Uninsured Farxiga/Xigduo XR patients should instead use the Farxiga Family Savings Card cash-pay path ($150/month off — see az_farxiga). (2) Brilinta removed from AZ&Me entirely 1/1/2026. (3) Onglyza + Kombiglyze permanently discontinued by AZ (drugs themselves, not just PAP). Eligibility: US resident, treated by US-licensed HCP, must NOT have any commercial or government insurance other than Medicare. Medicare beneficiaries: must NOT be eligible for or enrolled in Medicare Extra Help / Low-Income Subsidy (LIS). Cannot be receiving other medication assistance. Income limit ≤300% FPL for primary medications including Farxiga/Xigduo XR. Other diabetes-relevant AZ&Me drugs (non-diabetes): AIRSUPRA, BEVESPI, BREZTRI (COPD/asthma), LOKELMA (hyperkalemia).
